Cardiology Consultants accepts most insurance. If you have
a question about whether Cardiology Consultants is a member
of the physician panel that provides care for your insurance
plan, please ask at the time that you make your first appointment.
Since your account is ultimately your responsibility, it is
to your benefit to understand your insurance. You otherwise
may be responsible for a higher co-pay than was anticipated.
Payment of co-pays and co-insurance amounts is expected at
the time of service.
At the time of your appointment, we will ask you for your
insurance card. In the event that your insurance information
changes during the year, kindly notify us when you come to
the office. If any of your personal information, such as address
or phone number, changes during the year, please notify our
receptionist when you check in.
We will file insurance claims (primary or supplemental) as
a courtesy to you. If you are a member of an HMO or PPO, your
insurance company may require that you have a referral form
from your primary care physician before you are seen. If this
form is required, it must be presented at the time of service.
We accept various methods of payment, including cash, personal checks, money orders, Visa, MasterCard and Discover. Short-term payment plans are available in special circumstances. Our experienced business office personnel will be happy to discuss your insurance and payment needs. If you have no insurance, you are responsible for payment at the time of service for the procedures performed, unless prior arrangements have been made.
